NJ InCK

MyCareCircle is currently being utilized by the New Jersey Integrated Care for Kids program (NJ InCK), an innovative initiative aimed at enhancing care for children by focusing on prevention, early identification, and treatment of complex behavioral and physical health challenges. The NJ InCK program receives funding through a grant from the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of the Center for Medicare and Medicaid Innovation’s (CMMI’s) Integrated Care for Kids (InCK) initiative.

This successful implementation of MyCareCircle highlights its user-friendly nature, along with the integration of a consumer mobile app. The app enables youth, parents, and guardians to complete standardized mental, behavioral, and social welfare assessments. The collected information plays a crucial role in identifying and managing school-aged populations, while also connecting families to essential community resources through an integrated social care referral management system.

While the state of New Jersey has encountered significant barriers to improving care coordination for children with complex health needs, the NJ InCK program is paving the way for other states who are looking to improve care coordination and reduce out-of-home placement.
