CT InCK

MyCareCircle is currently being utilized by the Connecticut Integrated Care for Kids (CT InCK) – Embrace New Haven program. CT InCK is a locally focused service delivery and state payment model that prioritizes the well-being of children. Its objective is to reduce costs and enhance the quality of care by emphasizing prevention, early identification, and treatment of behavioral and physical health needs.

The Clifford Beers Community Care Center is a participant in the CT InCK program and has been chosen by the Centers for Medicare and Medicaid Services (CMS) as one of the recipients of the InCK Model award nationwide. This award aims to address the physical, mental, and social health requirements of Medicaid and Children’s Health Insurance Program (commonly known as HUSKY Health in Connecticut) enrollees. Spanning a duration of seven years, this initiative is part of CMS’ strategy to combat the opioid crisis and mitigate its impact on children, youth, caregivers, and expectant mothers. The model empowers states and local providers to effectively address these needs through improved care integration across various healthcare providers.

This award represents the extraordinary work of the staff at Clifford Beers and our passion toward our mission to improve the lives of children and their families. We are proud to be the local lead organization representing the State of Connecticut Department of Social Services in this incredibly important work. We hope to create an Integrated Care for Kids (InCK) Model in New Haven that can be replicated throughout the state.
